JOB SUMMARY

Guides and assists the patient through the complexities and barriers of the healthcare system.  Assists with all aspects of coordination of care.  Implementation and prioritization of patient’s problems and needs; and development/implementation/evaluation of a plan of care. Demonstrates critical thinking to assure the delivery of safe, quality, age specific care. Delegates effectively to appropriate team members. Documents accurately and in a timely manner. Communicates and collaborates with other healthcare team members. Adheres to organizational policies and procedures; regulatory accrediting body requirements; and professional practice standards.

    KNOWLEDGE AND SKILLS

    • RN: current knowledge of the nursing role and scope of practice as defined by the State of Ohio Nurse Practice Act and the Ohio Board of Nursing.
    • RN:  current knowledge of American Nurses Association (ANA) Code of Ethics.
    • Knowledge of adult learning concepts.
    • Knowledge of current trends and demonstration of standards of care in assigned unit.
    • Successful completion of mandatory requirements including unit proficiencies/competencies.
    • Demonstration of effective problem solving, decision-making, interpersonal, and teamwork skills.
    • Demonstration of effective verbal and written communication.
    • Demonstration of positive customer service.

    EQUIPMENT/TOOLS/SOFTWARE

    • Basic computer literacy (keyboarding; order entry; word processing)
    • Knowledge of and skill in using all instrumentation/ equipment for patient care in specific unit.

    PRIMARY JOB RESPONSIBILITIES

    • Develops and maintains relationships with the hospital and community service providers including American Cancer Society, Komen Foundation and other cancer social organizations.
    • Assesses/reassesses patient’s physical, psychosocial, and social needs based upon observation of patient and interview of patient and/or family, physician’s diagnosis and data, diagnostic results, and relevant data from other healthcare providers.
    • Uses professional knowledge to develop/revise plans of care for assigned patients according to unit guidelines and criteria with expected outcomes.
    • Delivers safe interventions in an organized, efficient, and prioritized manner to address identified needs.
    • Evaluates/re-evaluates patient’s response to interventions and patient’s progress to expected outcome(s).
    • Collaborates and communicates effectively with team members, physicians, and other healthcare providers to coordinate the medical and nursing plans of care. Acts as a liaison for patients who have been diagnosed with cancer.
    • Delegates responsibilities to appropriate personnel.
    • Documents all aspects of patient care and maintains patient records and care plans in a timely manner.
    • Provides patients and family members with relevant culturally targeted education and instructions regarding treatments and procedures including pre/post hospital care as appropriate.
    • Assist in the orientation and training of new and tenured personnel, reassigned nurses, student nurses, and faculty; serves as a preceptor when requested.
    • Demonstrates knowledge of disaster/ emergency procedures and responds appropriately.
    • Assumes responsibility and accountability for individual knowledge, skills, performance and behavior in accordance with hospital, division, and unit standards of care and policies and procedures.
    • Utilizes measures to promote and maintain patient, visitor and personnel safety.
    • Maintains patient, employee, physician, and organization confidentiality; respects the rights, privacy, and property of others.
    • Demonstrates the appropriate use of infection control principles and personal protection equipment.
    • Supports the mission, values, and vision of the organization.
    • Assists with the development and maintenance of an on-going initiative to improve health for the community.
    • Provides and assists patients and families to utilize available resources.

    Requirements Include

    EDUCATION AND WORK EXPERIENCE

    • Graduate of an accredited program for nursing education- BSN preferred.
    • Current licensure to practice professional nursing in the State of Ohio.
    • Current American Heart Association (AHA) Healthcare Provider CPR (BLS) certification required.
    • Clinical skills and expertise in the area of oncology preferred.
