JOB SUMMARY

Provides medical care services within the nurse practitioner’s scope of practice and consistent with the nurse’s education and certification as defined by the Ohio Board of Nursing, as well as in accordance with the privileges granted by the Board of Trustees.  Provides care to patients, independently and interdependently as stated in the Standard Care Arrangement with the collaborating physician(s).  Prescribes drugs and therapeutic devices in accordance with Section 4723.481 of the Ohio Revised Code.  Evaluates and promotes patient wellness. Assesses, plans, implements, and evaluates an appropriate plan of care including diagnostic and therapeutic interventions.  Collaborates with other physicians, healthcare providers, and others.    

    KNOWLEDGE AND SKILLS

    • Current knowledge of the Advanced Nurse Practitioner role and scope of practice as defined by the Ohio Nurse Practice Act and the Ohio Board of Nursing.
    • Current knowledge of the American Nurses Association (ANA) code of ethics.

    PRIMARY JOB RESPONSIBILITIES

    • Assesses the physical and psychosocial status of patients by means of interview, health history, physical examination, and diagnostic studies.
    • Recognizes deviations from normal in the physical assessment. Works in collaboration with a physician in formulating treatment plans for health problems and follow-up.
    • Writes prescriptions for medication, blood products based upon laboratory results, routine diagnostic and follow-up studies, therapeutic measures, and post-discharge care in accordance with written practice protocols.
    • Requests written consultation from physicians and other healthcare professionals to ensure appropriate and quality patient care.
    • Interprets and evaluates findings of studies/tests within the APN SOP.
    • Relays appropriate information regarding patient care to the collaborating physician.
    • Administers therapeutic measures and obtains specimens.
    • Evaluates the quality of care provided and recommends changes for improvement.
    • Initiates appropriate actions to facilitate the implementation of therapeutic plans that are consistent with the continuing healthcare needs of the patient.
    • Provides follow-up letters to referring physicians with summaries of treatment, response, and plan (when applicable).
    • Maintains provision of preventive health services (e.g. screening, risks assessment, immunizations, PAP smears, self-breast exam).
    • Documents patient history, admissions, care plans, progress, and discharge notes.
    • Assists in the management of family dynamics and coping mechanisms during acute and chronic phases of patient care.
    • Participates in multidisciplinary meetings/ communications and ensures appropriate and quality care for assigned patient.
    • Provides on-call coverage when necessary.
    • Performs patient triage and determines the urgency of physician’s evaluation.
    • Share clinical expertise with professional and clinical staff
    • Documents and reports care in an accurate and timely fashion based on the policies of the organization and department.
    • Demonstrates professionalism and accountability
    • Participates in patient centered medical home model
    • Provides comprehensive, coordinated care management through care team collaboration of all patients.

    • Participate in local, state, and national professional organizations
    • Participate in staff meetings, educational programs, committee, QI activities, and mandatory inservices
    • Maintain professional growth for self and incorporate evidence based knowledge into clinical practice through continuing education, conference, workshops, professional literature, and/or additional training including timely completion of HealthStream and other educational assignments.

    • Other duties as assigned consistent with RN or APRN-CNP training and licensure.
    Requirements Include

    EDUCATION AND WORK EXPERIENCE

    • Current RN licensure to practice nursing in the State of Ohio
    • Graduate from an accredited program for Advanced Practice Nursing
    • Board certification from an approved national certifying organization for Adult Nurse Practitioner through the AmericanAcademy of Nurse Practitioners (AANP) or the American Nurses Credentialing Center (ANCC).
    • Current Standard Care Arrangement with collaborating physician(s)
    • Current unrestricted Certificate of Authority (COA) from the Ohio Board of Nursing to practice as a certified Advanced Practice Nurse
    • Current unrestricted Certificate to Prescribe (CTP or CTP-E for the newly graduated NP – the “E” stands for externship and has requirements to meet in order to get a CTP) from the Ohio Board of Nursing  
    • Current American Heart Association (AHA) Healthcare Provider CPR (BLS) and/or Advanced Cardiac Life Support (ACLS).
    • Medical Staff privileges are a prerequisite for hire and continued employment
    • A minimum of three (3) years of clinical nursing experience.
    •  It is the sole responsibility of the APRN-CNP to assure that the privileges granted by Knox Community Hospital or its affiliates and the practice the APRN-CNP engages are consistent with the scope of practice requirements imposed by the Ohio Board of Nursing and certification organizations.
