To schedule or cancel an appointment, please call 740.326.3531. Convenient appointments are available. When canceling an appointment, please call as soon as possible so we can offer the time to another patient.
Requests for prescription re-fills must be received 72 hours in advance. Please call 740.326.3531 and include the following in your message: your name, your date of birth, the medication you are requesting and pharmacy you would like the prescription sent to. Please contact your pharmacy regarding your prescription, as the office will only contact you if there is an issue with your request.
Our hospital accepts all major insurance carriers. It is the patient’s responsibility to pay any applicable co-payment on the day of your office visit. Please bring your insurance card with you each time you visit.

Both require monthly payments with total cost paid in full by week 32. If amount is not paid in full by week 32, the self-pay package price would not apply and would default back to regular pricing.
